ABOUT LIFEBASE care

LifeBASE Care is our transitional program for pregnant or parenting youth ages 18-24. We equip pregnant and parenting youth for a successful transition into adulthood for themselves and for their young families by providing housing, one-on-one coaching, mentoring, life skills training and more.

We walk with young parents through every step of the journey. Youth will have access to supportive housing, one-on-one coaching, life skills training, parenting training, educational and employment support, mentoring and a 24/7 crisis helpline.


POSITION ROLE DESCRIPTION

Reporting to the LifeBASE Manager, the LifeBASE Care Coach will provide transitional coaching, support services and independent living skills training to pregnant and parenting youth in IA’s LifeBASE Care program, using an interdisciplinary approach. In addition, the role will provide:

1. Coaching - Support youth from crisis and their dependants in the LifeBASE Care program, walking with them as they achieve sufficiency in the areas of Well Being, At Least One (Healthy Relationships), Stable Housing, and Earning Potential, effectively utilizing Immerse Arkansas’ Overcomer process in a way that upholds Immerse values and the principles of Trust Based Relational Intervention.

2. Coordination of Care Services - Successfully manage referrals for mental health services and provide high level support to include assessing for social, emotional, physical and mental health impairments; when relevant refer to outside providers to address identified needs.

3. Youth Enrichment & Parenting Support - Provide opportunities for positive interaction, healthy connection, growth and fulfillment through consistent youth-centered activities and events.

4. Cross-Program Support - In cooperation with the entire Immerse Arkansas team, create a safety net throughout the various programs of Immerse to ensure all youth receive the best possible care and support, responding timely and appropriately to individual needs.

5. Participate in Team and Personal Development - Continually seek opportunities for growth, understanding, and balance to maintain health, both individually and as a member of the Immerse Arkansas team. Promote teamwork through collaboration, support, and communication.

QUALIFICATIONS

Education: Bachelor’s degree in social services or related field preferred.

Experience: Three or more years of working with youth who are at-risk or in foster care. Two years or more in effective case management and/or integrative behavioral health experience.

Additional Requirements: 

  • Understanding of adolescent development; comfortable interacting with youth in an

    informal setting.

  • Must be available evenings, weekends and on-call as assigned, during identified days.

  • Must successfully complete the required background checks and drug screening.

  • Adherence to an ethical code as it relates to the particular professional field.

  • Strong people skills. Able to form effective working relationships with a wide range of people.

  • Strong organizational skills and ability to drive multiple simultaneous projects to completion.

  • Wants to do “whatever it takes” to see youth and families succeed.

  • Have experience working with homeless transitional age youth.

  • Must demonstrate a commitment to the value of cultural competency.

  • Must be able to lift 35 pounds.

  • Must have a valid driver’s license and reliable transportation.

  • Must be willing and able to travel within the assigned region on a daily basis.

  • Must maintain current certification in CPR, First Aid, and other certifications as determined by the program.

Immerse Arkansas was founded in 2010 with the mission of helping transform youth from crisis into overcomers. The nonprofit organization serves youth between the ages of 14 and 24, whose past trauma and challenges have created barriers to becoming independent, healthy, successful adults.
